Early breastfeeding practice in West Sumatra, Indonesia : determinants and interventions for promotion

Abstract: BACKGROUND: Early breastfeeding which initiated within one hour after birth have been associated with reduction of neonatal and infant mortality. Thus contributed to under five mortality. Therefore, giving the breast milk as early as possible is important for the baby’s growth and development. OBJECTIVE: This paper will identify the determinants and reviews the program on early breastfeeding and the intervention that already taking place in Indonesia, particularly in West Sumatra. It will also see the possibilities to adopt other best practices on early breastfeeding from other Low Middle Income Countries (LMIC) in Indonesia, especially in West Sumatra. METHODS: Literature review will use Lutter’s model (2000) as an analytical framework to assess determinants that influence early breastfeeding practice. While, Wellstart International (1996) cited in WHO (2003) as analytical framework to assess intervention for early breastfeeding. RESULTS: Determinants, Most of the determinants mentioned by Lutter’s model influence the early breastfeeding practice in Indonesia including in West Sumatra. The main determinants are: medical, cultural attitudes and norms, demographic and economic condition, commercial pressure, policy, feeding information and support system. Interventions, The national regulation on early breastfeeding is still on draft now. Therefore, the intervention guideline for early breastfeeding is not clear in province level. In West Sumatra, only 5 out of 40 hospitals adopted BFHI. Only four health workers were trained about early breastfeeding since 2007. Health workers who are not trained have different perception about early breastfeeding even on the definition of early breastfeeding. Early breastfeeding intervention in community level is absent. Learning from the experience in Sri Lanka, it is difficult to adopt the best practices at policy level in Indonesia before the government change the decree become a law. In West Sumatra, revitalization of hospital which accredited with BFHI can be applied. In the other hand, early breastfeeding training for health workers is difficult to apply due to limited budget. Monitoring and evaluation of early breastfeeding which involve interview with mother can be pertained. CONCLUSION: All of the intervention on the community and Information, Education and Communication (IEC) program that succeed in Bolivia, Madagascar, Bangladesh, Guatemala and India can be adopted in West Sumatra. To support implementation the central government should legalized the draft on breastfeeding national regulation, and the local government should allocated special budget for the implementation of the program.
