You can obtain mortality data in the US from the CDC Wonder , the database on the Internet, maintained by the Centers for Disease Control and Prevention. This database allows one to obtain mortality in the US by age-group, race, sex, state, county, underlying cause of death by the International Classification of Diseases codes ninth revision (ICD).

In comparing the mortality from CHD within the US, ICD codes 410-414 plus 429.2 were used. This is because in the US, code 429.2 is exclusively and extensively used by coroners to denote CHD that falls somewhat short of definitive evidence of a myocardial infarction, but was separated from the rest of codes for CHD in the ninth revision of ICD. Such deaths were coded 412.4 under the ICD eighth revision. ICD code 429.2 was not used for international comparison because it is not available from the World Health Organization Health Statistics Annual.
