After ingestion, the sporozoites from the cyst penetrate vascular endothelial cells of the intestinal tract where they reproduce asexually (merogony) to produce merozoites which migrate to the central nervous system, where further asexual reproduction occurs intracellularly in neurons.

The disease cannot be spread directly from horse to horse.
