Indigenous control of health services should be applied both in terms of community controlled health services (see below) and for Indigenous sections within mainstream health agencies. There needs to be Indigenous participation at all levels of health service policy, administration and practice. Outside the community controlled sector, representation by Indigenous people is disproportionately small.

Self determination is seen to be a necessary foundation for any truly sustainable improvement in Indigenous health. (O'Donoghue 1999)
