I went through
versions 3, 5 and 7. For the BMJ version, I printed out a hard copy of the
PDF version. I still find that the easiest to read. The Web version of the
PowerPoint presentation with sound has great promise, but I suspect many
users may still encounter some technical limitations. I don't have speakers
on my office computer, for example, and your oral commentary was rather soft
from my little built in speaker. I had to crane my neck to hear it. Gerrit
